Here is an authorization form for automatic monthly withdrawals from your checking account, or your credit or debit card, to pay for Giant Eagle gift cards. Return the completed form to Temple and keep a copy for your records. You should indicate the amount to be withdrawn on the form. A $200 monthly minimum is required for automatic withdrawals. Please identify the day of the month, either the 1st or the 15th, on which you wish the withdrawal to be made. All information is strictly confidential.
